Assumption of Risk

I, the undersigned parent or legal guardian, understand that participation in soccer training, games, and activities organized by Little Dreamers Soccer Academy (LDSA) involves certain inherent risks of physical injury, including but not limited to sprains, strains, fractures, concussions, heat-related illness, and, in rare cases, more serious injury.

By registering my child, I voluntarily assume all risks associated with their participation in LDSA programs, whether on or off the field, including transportation to and from events.

Waiver of Liability

In consideration for allowing my child to participate in LDSA programs, I hereby release and hold harmless Little Dreamers Soccer Academy, its directors, coaches, volunteers, staff, affiliates, and facility partners from any and all liability, claims, or demands for damages arising out of or relating to injury, illness, or property loss that may occur during participation, except in cases of gross negligence or willful misconduct.

Medical Authorization

In the event of an injury or medical emergency, I authorize LDSA staff or its representatives to secure medical treatment for my child. I acknowledge that I am responsible for all medical expenses and waive any claim against LDSA for such treatment provided in good faith.

Media Release

I grant LDSA permission to use photographs, videos, or likenesses of my child for promotional, marketing, and educational purposes in both online and print media.

Code of Conduct

I agree that my child and I will adhere to the academy's Code of Conduct, respecting coaches, teammates, referees, and facilities. I understand that failure to do so may result in dismissal from the program without refund.

Refund Policy Acknowledgement

I have read and understand the academy's posted refund policy. I acknowledge that refunds are not guaranteed and are subject to conditions outlined on the website or registration agreement.

Agreement and Signature

By signing below, I certify that I am the parent or legal guardian of the minor named above and that I have read, understood, and agree to this waiver of liability and registration terms.
